A Study of the Effect of Sbort-term Prophylactic Antibiotics for the Poeration of Clean Contaminated Wound




Abstract
The incidence of post-operative infectious complications after cholecystectomy and gastrectomy was investigated in 70 patients included in a prospective study. The study group was divided as follows.
Group A was not given any antibiotics.
Group B was given 1 gm intravenous dose of first generation Cephalosphorin(Cefazolin) every 6 hours and 120 mg intramuscular dose of Micromicin every 12 hours for 1 day.
Only elective operation was included and the patients susceptable to infection were excluded. Out of the 70 patients, 35 did not receive antibiotics(Group A).
Analalytic results of clinical evaluation untill 30th days after surgery showed that over infectious complication rate was 22.9% in 70 patients-7 cases(20%) in gvoup A and 1 case(2.9%) in group B. There was a statistical significance between
group
A and
group B. The two groups of patients were similar in regard to age and sex in complication rate. The most common complication wao wound infection.
The organisms isolated from swabs taken from infected wound were Staphylococcus, Pseudomonas aeruginosa, Citrobacter, and Streptococcus pyogenes. The subcostal incision showed the highest rate of wound infection. There was no wound infection in
group B.
Therefore snort-term antibiotic prophylaxis was effective to reduce a postoperative infectious complication.
